What is not on this list
Flights, kennels and any charter or ferry leg you take to reach Utila — those vary far too much to estimate usefully. The traveling to Utila page covers the two realistic routes and roughly what each involves, and the ferry page has current ticket pricing.
Airline pet fees are the other big variable. They differ by carrier, by cabin versus cargo, and by whether your dog is a service animal — check directly with your airline before you assume a number.
